What is Vitamin B12?

It is produced by soil-dwelling bacteria that accumulate in the tissues of animals. For those following a plant-based or vegan diet, obtaining adequate amounts requires fortified foods or reliable dietary supplements.

Once consumed, the absorption of vitamin B12 is a complex, two-step process. First, hydrochloric acid in the stomach separates the vitamin from the protein to which it is attached. Next, the vitamin combines with a protein made by the stomach lining called intrinsic factor, which allows it to be absorbed further down in the small intestine.

As we age, or due to certain digestive conditions, the stomach produces less acid and intrinsic factor. This can significantly reduce the body's ability to absorb B12 from food, making deficiency a relatively common issue, particularly among older adults and those on strict plant-based diets.

While regular intake helps keep these levels within a healthy range, clinical trials have not yet proven that reducing homocysteine directly translates to a lower risk of heart disease.

In the United Kingdom, the recommended dietary allowance (RDA) for adults is 1.5 micrograms per day, though many supplements contain much higher amounts because the human body only absorbs a small fraction of oral doses. Vitamin B12 supplements are commonly available in two main forms: cyanocobalamin, a stable and cost-effective synthetic form, and methylcobalamin, a naturally occurring active form. Daily oral tablets, sublingual spray under the tongue, or fortified foods like nutritional yeast and cereals are all effective ways to maintain healthy levels.

Safety and who should avoid it

Vitamin B12 is generally considered exceptionally safe, even at high doses, because it is water-soluble, meaning the body excretes any excess through urine. However, certain groups should exercise caution and consult a healthcare professional before starting high-dose therapy. Individuals with conditions like Leber's hereditary optic neuropathy (a rare eye disease) or those taking specific medications—such as proton pump inhibitors for acid reflux or metformin for diabetes, which can interfere with absorption—should have their levels monitored by a GP.
